Patient Advocacy and Care Navigator - Full Time - Vibra Health Plan
Location: Vibra Health Plan - Harrisburg, PA

Vibra Health Plan (VHP) is seeking a Full Time Patient Advocacy and Care Navigator to join our team!

The Patient Advocacy and Care Navigator staff will guide members to achieve optimal and vibrant health by providing tools, information, and assistance to help:

•Understand their healthcare options,
•Take control of their healthcare needs,
•Bridge the current gap between social-economic and medical needs, and
•Navigate the otherwise often confusing steps along the path to efficient and effective care.

The Patient Advocacy and Care Navigator staff will work in collaboration with all VHP management and staff to achieve the goals of the organization and the Patient Advocacy and Care Navigator program. Staff will report directly to the Director of Utilization and Care Management.

Assist VHP members with navigating the otherwise often confusing steps along the path to efficient and effective care.
Work with VHP multidisciplinary teams to develop communication, case management, and patient outreach plans; assessment of goals and outcomes.
Identify and assess patient medical, social, emotional, and financial needs.
Assist VHP members in understanding their healthcare options.
Effectively and efficiently utilize the proprietary VHP database of social, economic, behavioral, and support systems and programs to connect at-risk members with appropriate community resources to address barriers and adherence.
Establish and maintain communication with patients, families, healthcare decision-makers, physicians, and provider systems.
Coordinate appointments with and transportation to physicians and non-physician providers to ensure timely and efficient delivery of diagnostic and treatment services when needed.
Assist with all transition of care needs (e.g. medications, meals, DME, transportation)
Access and facilitate financial support and social services as necessary.
Provide emotional support and/or referring to community-based or physician/provider for greater level of psychosocial intervention.
Assist with patient-physician/provider communications.
Coordinate and communicate with VHP clinical and customer services.
Other duties per patient needs.
Complete all required compliance programs and continuous training and education, including department specific requirements.
Performs other related duties as assigned.

Bachelor’s degree in Social Work, Psychology, or Nursing field required.
Ability to communicate effectively, both orally and in writing, with groups from diverse backgrounds and technical abilities.
Excellent customer service mentality and experience required.
Minimum three (3) years of Social Work or Care/Case Management experience preferred.
Minimum three (3) years of experience working with patients with Medicare preferred.
Patient Navigation certification preferred.
